Rate assistance is offered only to uninsured patients or those whose insurance benefits do not cover alternative medicine. Fees must be paid in full with cash at time of service. Income-based special rates require two forms of income verification. There is no rate assistance option for patients whose insurance covers natural medicine. Eligibility will be confirmed by patient services representatives, verified by income statement and reviewed semi-annually from the date of enrollment. Accepted forms of income verification: (for current pay period) pay stub, bank statement, disability award or income tax form signed by preparer and filed with IRS. Patients with permanent disability status are exempt from semi-annual review.
